Credit Card Authorization Form

The CLDV credit card authorization form is to be filled out and on file before rental of the camp facilities.  The credit card authorized on file can be charged for incidentals, cancellations and/or payments.  ​ You may cancel this authorization at any time by contacting us. This authorization will remain in effect until cancelled.
